Pieni Apu On Suuri: Kulttuurien yhteentörmäyksiä ja koulutuskuvia / Cultures collide and pictures of the training

Joskus vieraassa kulttuurissa ollessa tulee eteen hyvin odottamattomia, jopa ikävältä tuntuvia tilanteita. Tilanteita on vaikea estää, vaikka yrittäisi olla hyvin kohtelias ja ymmärtäväinen. Olimme Mawensin sairaalan johtavan lääkärin sekä ylihoitajan luvalla aloittaneet painehoidon pienelle palaneelle tytölle. Mennessämme muutaman päivän jälkeen tapaamaan urheaa potilastamme, kohtasimme jotain odottamatonta.

Osaston ylilääkäri tuli meidän luoksemme hyvin tuohtuneena. Hän kertoi harmistuksestaan, ettei aloittamamme hoito ollut auttanut ja että hän ei halua meidän jatkavan painehoitoa. Tytöltä oli otettu painevaate pois. Yritimme kertoa hänelle, että olemme pahoillamme ja että olimme keskustelleet asiasta edellisellä kerralla paikalla olleen lääkärin sekä sairaalan johtavan lääkärin kanssa. Valitettavasti osaston ylilääkäri ei ottanut vastaan kertomaamme.

Hieman hämillämme lähdimme etsimään ylihoitajaa, koska halusimme tietenkin heti selvittää tilanteen. Keskustelimme pitkään ylihoitajan sekä osaston muiden hoitajien kanssa. He olivat hyvin pahoillaan tapahtuneesta, mutta hoitajat olivat tyytyväisiä hoidosta. Harmiksemme tässä oli käynyt tietokatkos ja täten ylilääkäri ei ollut saanut tietoa meidän olemassaolosta ja tekemästämme työstä. Hän oli syystäkin harmissaan, koska tavallaan astuimme hänen varpailleen, koska hän on vastuussa potilaan hoidosta ja ei tiennyt, mitä oli meneillään. Onneksemme tapasimme vielä tytön ja näimme, että hoito oli kyllä auttanut.

Myöhemmin vielä Tanzania Volunteersin Louis oli yhteydessä sairaalaan ja keskusteli asian läpi johtavan lääkärin kanssa. Sairaalan henkilökunta oli hyvin pahoillaan tapahtuneesta ja meille aiheutuneesta harmista, ja samalla he olivat hyvin kiitollisia jakamastamme tiedosta ja hoitajat fysioterapeutti Katarinan kanssa jatkavat painevaatteiden käyttöä myös Mawensissa. Onneksi asioilla on tapana järjestyä.

Sometimes unexpected, and even sad, situations come up when you’re in a different culture. These are hard to prevent even with the utmost of understanding. With the consent of chief senior physician and senior nurse at Mawezi hospital we started pressure treatment on a little girl with burns. When we went back to check on her after a couple of days we were faced with something unexpected.

The ward’s senior physician came to us heavily upset. He told us that the treatment has not helped and that it will not be continued. The girl was stripped from her pressure garment. We tried to tell him that we’re sorry and we had permission to treat this little girl. Unfortunately he was not reached by pleads.

Confused, we went to search for the senior nurse to clarify the situation. We had a long talk with the senior nurse and other nurses at the ward. They were very sorry since they were happy with the treatment. Turned out that there had been a lapse in communication and the senior physician was not notified about our existence and the work we do. He was mad for a reason, we were kind of stepping on his toes and the responsibility for the patient’s care was ultimately his. Luckily we met with the girl and saw that the treatment had had effect on her.

Later Louis (from Tanzania Volunteers) contacted the hospital and discussed the matter with the chief senior physician. The hospital staff was sorry about everything and very grateful about the information we have shared with them. The nurses along with Katarina (the physiotherapist) will continue using pressure garments in Mawenzi as well. Luckily matters tend to sort themselves out.
